Sub-Saharan Africa(SSA) has an additional burden of disease: The health systems must cater for both communicable and non-communicable conditions including cardiometabolic conditions such as atherosclerotic cardiovascular disease, diabetes, and hypertension. Unfortunately, sub-Saharan health systems are relatively less robust and less responsive to non-communicable diseases than to communicable conditions.

 

The increasing prevalence of multi-morbidity and the clustering of cardiometabolic conditions in individuals necessitates the use of integrated models of care for delivering care to people with cardiometabolic conditions.

  • The Chronic/Collaborative model is an integrated model of care developed specifically for managing cardiometabolic conditions in resource-constrained settings in SSA by the NIHR GLOBAL HEALTH RESEARCH GROUP ON COLLABORATIVE CARE FOR CARDIOMETABOLIC DISEASE IN AFRICA: CARDIOMETABOLIC DISEASE RESEARCH IN AFRICA PARTNERSHIP (CREATE).

Aim of study: To conduct a feasibility study evaluating a structured self-management education (SEM) intervention for people living with cardiometabolic disease (CMD).

SPECIFIC OBJECTIVES
1. To determine the association between structured self-management education (component of the Chronic/collaborative model) and selected markers of cardiometabolic disease in people with chronic cardiometabolic disease.

  1. To pilot the fidelity framework for the structured self-management education component of this chronic/collaborative model, (developed by the CREATE partnership) in community and hospital settings.
  2. To understand the extent to which the structured self-management education component of this model is delivered as conceived and planned

This is a feasibility study currently ongoing at the Create Ghana study site in Korle Bu Family Medicine Department/polyclinic. This study is an NIHR-funded study being conducted in 3 different West African countries. This includes Ghana, Kenya, and Mozambique.
