Sub-Saharan Africa(SSA) has an additional burden of disease: The health systems must cater for both communicable and non-communicable conditions including cardiometabolic conditions such as atherosclerotic cardiovascular disease, diabetes, and hypertension. Unfortunately, sub-Saharan health systems are relatively less robust and less responsive to non-communicable diseases than to communicable conditions.
The increasing prevalence of multi-morbidity and the clustering of cardiometabolic conditions in individuals necessitates the use of integrated models of care for delivering care to people with cardiometabolic conditions.
- The Chronic/Collaborative model is an integrated model of care developed specifically for managing cardiometabolic conditions in resource-constrained settings in SSA by the NIHR GLOBAL HEALTH RESEARCH GROUP ON COLLABORATIVE CARE FOR CARDIOMETABOLIC DISEASE IN AFRICA: CARDIOMETABOLIC DISEASE RESEARCH IN AFRICA PARTNERSHIP (CREATE).
Aim of study: To conduct a feasibility study evaluating a structured self-management education (SEM) intervention for people living with cardiometabolic disease (CMD).
